Changes to be Reported
A.
Require each assistance
unit to report changes by phone, in person, in writing or on the
change report form (DFS 103) during regular business hours.
B.
Changes to be reported include: 1.
Changes in assets,
excluding for Child Care, EDI,
Family Care and Kid Care A/B;
2.
Changes
in marital status, school enrollment and allowable expenses; 3.
Changes in assistance
unit size, living arrangements and address; 4.
Changes in earned income
such as source of income,
beginning/ ending employment,
rate of pay and increase/decrease in number of work hours; 5.
Changes in unearned income; 6.
For Child Care, changes in the current or benefit month’s conditions
such as a change in provider
or other situations which could affect the benefit and parental
obligation; 7.
For Medicaid, change
in health insurance coverage; 8.
Other changes in circumstances. C.
DOCUMENT the change and action taken in the
case record.
D.
Changes to be reported include: 1. Assets
when exceeding the asset limit; 2. Assistance
unit size, living arrangements, address and a change in shelter costs
due to the address change; 3.
4.
Changes in unearned income
of $50 or more; 5.
Changes in legal obligation
of child support payments,
i.e., ceasing payment, increase, decrease, etc.; |
